Facilitating Change in Health and Social Care appointed as the new Manager of Hounslow West Care home or the care home where you work. This is a care home for the elderly between the ages of 40-90 years with dementia. The setting is a 25 bed home with 24 hours care delivery. The home is failing in its care responsibilities because of various reported incidents of clients’ abuse, health and safety concerns and other issues of great concerns that you have identified. With your knowledge and skills acquired during your level 5 course, you feel the urgent need to effect changes in the setting so as to deliver a high standard and qualitative service for your clients and their families. In addition, you have received a letter confirming a visit by the Care Quality commission (CQC) within the next three (3) months.
You are required to write a detailed plan and explaination to convince all stakeholders of the care home within the next three months how you will bring about changes to address the identified faillings, so as to bring the care home up to an acceptable standard. Your plan should include the following information:
