According to the United Kingdom Central Council, advanced practice is defined as “adjusting the boundaries for the development of future practice, pioneering and developing new roles responsive to changing needs and with advancing clinical practice, research and education enrich professional practice as a whole” (UKCC, 1994: 20). In contrast, The International Council of Nurses define advanced practitioner as a registered nurse who has acquired the expert knowledge base, complex decision making skills and clinical competencies for expanded practice, the characteristics of which are influenced by the context and/or country in which she or she is credentialed to practice (McGee, 2009).

 

The essay is divided into two parts. The first half of the essay discusses the concept and development of advanced nursing practice in the UK since 1970s whilst focusing on the critical evaluation of the nature and conceptualization of advanced and specialist practice. The second half looks at the ways in which, leadership can be used to advance the leadership practice, alongside the use of the other three pillars (xx, xx and xx) to help promote advanced nursing practice.

 

Jasper (2006) argues that the development of advanced nursing practice in the UK has been poorly documented and remains fragmented however, the evidence available points to the acceptance of the notion during the 1970s that alongside the progression of clinical nurse specialist roles in 1970s, the idea gained acceptance that some nurses were able to undertake more demanded work relative to others. This was followed by subsequent reforms in 1980s in nurse education, which increased the professional debate relating to the levels of practice and the requirement to aim towards practitioner-focused activity and making use of division of labour to provide flexibility during the time of rapid change in the external environment (Barker, 2009).

 

Advanced nursing began to be viewed as a separate area within nursing instead of simply an extension to the specialist role whereby nurse becomes more specialist and knowledgeable. McGee et al (1996) specified seven key criteria, functions and roles of the advanced nursing practice applicable in the UK during mid-1990s. These included autonomous practitioner, expert in case management, expert in health and nursing assessment, knowledgeable and experienced, researcher and valuator of care, consultant and leader, and respected and recognized by others in the profession. Significant changes in the environment including the changes in healthcare, development of new clinical roles (e.g. nurse practitioners) and the rise in specialist nurses in late 20th and early 21st century facilitated the development of initiatives such as midwife and nurse consultants.

 

Thus it is clear from the above that the development in nurses’ roles, and the responsibilities inherent in the roles has been largely haphazard and lacking a planned coordination amongst various stakeholders. The roles developed to meet local needs actually contributed to heightened confusion relating to role boundaries, provision of appropriate training and titles.
